Medical Records Requests

We strive to make access to your medical records as easy as possible for the continuation of your care. To request a copy of your medical records you will need to complete an Release of Medical Information Form. Once that authorization has been received, the request will be completed and mailed to the provider as stated on the authorization within 14 days.

Birth Certificate Program

We offer complimentary Hospital Birth Certificates to babies that are born at North Valley Hospital. Although this is not a legal document, it provides a family record of the child’s birth, which is complete with a copy of the photo that is taken for the “Baby’s First Web Page”.

Legal Certificates of Birth for all children that were born in Washington State after 1939 are available through the Okanogan County Health District. For information and pricing please access this website: www.okanogancounty.org/ochd. If a child was born prior to 1939, you can access the Center for Health Statistics in Olympia at this website: www.vitalchek.com.

We are dedicated to keeping your medical information confidential, which is why we need a completed and signed Release of Medical Information Form in order to release your records.
