Flu Vaccine REMINDER
100% Documentation of either proof of receiving the flu vaccine or a signed declination is required. Cassi reported we are just shy of 40% documention completed.
Fluzone Quadrivalent vaccinations for the 2025–2026 flu season are now available at the following locations:
LTC East Nurses’ Station
LTC West Nurses’ Station
Acute Care Nurses’ Station
Cassandra's Office
Each site includes:
The Employee Flu Vaccination Binder
Consent and declination forms
CDC Vaccine Information Statements (VIS)
Please place completed vaccination records and declination forms in the folder located inside the binder.
